B*LEAGUE | WAIVER AND RELEASE
I, [printed name], a participant in the League (the "League", which includes traveling to and from the "League") hereby understand/agree with the following:
- That participation in the League is a potentially hazardous activity and can result in serious injury or death. I am aware of and I expressly assume all inherent risks associated with participation in the League.
- That I was given the opportunity to inquire about the risks and dangers involved in participating in the League and that these risks were completely and clearly explained to me, and thus I fully understand the same;
- That I, for myself and anyone entitled to act on my behalf, waive and release the B*League and I will release and hold the B*League, Vegas Aces, all its vendors, staff, and organizers harmless from any and all liability, claim, loss or lawsuit that may arise by reason of my participation in the League.
- That I attest that I am physically and mentally fit and have sufficiently trained for participation in the League.
- That I consent to receive medical treatment, which may be advisable in the event of illness or injuries suffered by me during the League, and I agree to pay for the costs of my medical treatment.
- That any and all COVID-19, or SARS-Cov-2, precautions, standard procedures, and proper hygiene will be observed and am subject to removal from participating and release the B*League from any and all liability, claim, loss or lawsuit that may arise by reason of my participation in the League
- That the foregoing is intended to be as broad and inclusive as is permitted by the law of jurisdiction and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
I have read and voluntarily sign this declaration
